Bundling
For physician services, there are components which are always performed during a service and should not be reported separately. The CPT codes are reimbursed with a fee that includes each piece needed to complete the entire service.

  • Example:
    An open reduction with internal fixation of a fracture is performed. An incision is used to open the area and after the reduction is complete that incision is closed. The wound being opened and closed is always a part of this service; as a result, you do not report the incision, suturing or wound closure as a separate service. It is included in the fee for the open reduction of the fracture.
The same applies for services like prepping and draping before surgery, checking for hemostasis during a case or an exploratory laparotomy with intestinal resection. The exploration of the surroundings is considered part of the intestinal resection, likewise exploration is included with any other procedure and you would not bill for it separately.
