
Modifiers
Because each of the procedures and services can be so diverse with each encounter, modifiers are used in helping to contour the codes to better fit each encounter. You may do more or less than the code describes; you could change one of the techniques within the description; you may perform a E&M service that would typically bundle into a procedure but is separate for this specific encounter or the service may be performed bilaterally.
These are just a few of the things that must be taken into account when choosing a code and/or a modifier if needed. Using modifiers correctly is the only way to avoid a possible fraud or abuse situation.
